Covid-19 Vaccinations for 12-15 year olds

This November, Covid-19 vaccinations for students aged 12-15 will take place at Stratford School Academy.
Vaccinations for children aged 12 to 15 years of age will take place on the following dates:
Grosvenor Road
Friday 12th November 2021
Upton Lane
Tuesday 16th November 2021
Vaccination Consent
All parents/carers must indicate their consent by completing the online e-consent form.
The link to the online e-consent form can be found within the vaccination letter below:
Please note that the cutoff for completing the consent is 2 days before the scheduled session, so we kindly ask that you complete this as soon as possible.
If you do not wish for your child to have the vaccine, it is important that you still use the e-consent platform to decline the vaccine.
